I declare that the information I have provided is correct and complete and understand that if I have made a false statement or have omitted any relevant and appropriate information I may be subject to investigation which could ultimately result in criminal proceedings.

I understand that the information I have supplied in this application may be shared, as the law allows, for the purpose of preventing and detecting fraud and corruption and that the offer of accommodation may be withdrawn and I may face criminal proceedings if my application is found to be fraudulent.

You will use the information I have provided to process my claim for Housing or any other financial assistance provided by the Council.

You may check some of the information with other sources (e.g. The Department for Work and Pensions) as allowed by the law.

You may use the information I have provided for the administration of Council Tax, Business Rates and council house rents.

I understand that you may use information I have provided in connection with this and any other claims for social security benefits that I have made or may make. The Authority is under a duty to protect public funds and may use the information I have provided for the prevention/detection of fraud. It may also, for these purposes, share the information with other bodies responsible for auditing/administering public funds. I know I must let the council's homeless team know immediately about any changes in my circumstances which might affect my application. I understand that if I do not tell you about any changes, you may take action against me. This may include court action and/or withdrawal of the offer of accommodation.

I declare the information I have given on this form is correct and complete. I give my permission for you to check information with any other sources that are relevant to my claim.

Please notify us of any changes yourself, do not rely on other organisations to do so.

I understand that if I have made any false statements, or have omitted any relevant information to try to obtain housing deceitfully then my name will be removed from the housing register; and any property allocated to me may be repossessed.
